Subject: Employee Survey

Greetings. 

We are conducting a confidential and anonymous survey that assesses the social atmosphere at work. The survey results will be analyzed by Changeculture.com, an independent consulting firm based in Burlington, Vermont. The survey should take  5-10 minutes to complete. Survey results will be made available in the (ABC Company Newsletter or through some other communication vehicle such as a workshop).

Please complete the survey online by October 31. You will be asked for a password. The password you should use is ABC. The survey is available at www.changeculture.com/Surveys/wcsurvey.asp.
