Greetings, 

We are conducting a confidential and anonymous survey that assesses the social atmosphere at work. The survey results will be analyzed by Changeculture.com, an independent consulting firm based in Burlington, Vermont. The survey should take  5-10 minutes to complete. Survey results will be made available in the (ABC Company Newsletter or through some other communication vehicle such as a workshop).

Please complete the enclosed survey by October 31. Your completed survey should be returned by interoffice mail to John Smith, Director of Human Resources, Room 120.
